This 2015 tax season, a number of new forms may have been required of you to either complete and file and/or retain for your records. Of these forms you should retain include Form 1095-A. You should have been mailed Form 1095-A if you or someone from your household obtained health coverage via the Health Insurance Marketplace in 2015. Upon eligibility, the information within Form 1095-A can be used to receive your premium tax credit. However, you may have received a letter regarding Form 1095-A or an entirely new Form despite being previously mailed one. Here’s why.

After the Form 1095-A was mailed to individuals and/or families, certain factors caused margins of error and the forms were either corrected or voided. If you’ve received a new Form 1095-A, simply view the top right of the form. There are two boxed: One reads VOID and the other CORRECTED. If one of those boxes is checked, then your Form 1095-A has been changed. Further, you may receive a letter from the Health Insurance Marketplace advising that your form was voided and to disregard the original Form 1095-A.

Corrected forms could have resulted from a number of different reasons. A corrected address is a simple correction and typically requires no further action from the individual. In contrast, if your premium amount has been modified, that is a complex correction that could change your return. If you haven’t yet filed your tax return, simply use the information on your corrected Form 1095-A for filing. If you have already filed your taxes with the original form, it’s best to consult with a tax professional, who will advise if your return requires an amendment. For questions regarding your corrected 1095-A or if you feel these corrections were made in error, reach out to the Health Insurance Marketplace.

Voided forms or letters advising of a voided form reflect an erroneous issuance of a Form 1095-A. This can be due to various factors, one of which is incomplete enrollment for health coverage through the Health Insurance Marketplace. If your Form 1095-A is voided, it cannot be used to file with your tax return. If it was filed prior to being voided, it may be best to file an amended return. Contact a tax professional for further information. Should you have a voided form, yet you did obtain coverage from the Marketplace, it is urgent that you contact the Marketplace for a correct Form 1095-A.
